• The forms can’t be saved or signed (Adobe signature embedded in the PDF or annotated via Apple Preview) on a mobile device or in a regular browser, so please download and complete them on a desktop.

• If you can’t complete them beforehand, I will provide a way for you to sign digitally when you arrive at the studio. 

• An intake form is legally required. Only one form is necessary unless otherwise requested later. You may email the completed form (preferred) to zdgraney@icloud.com or bring a printed copy to your appointment.
