• The forms cannot be saved or signed (Adobe signature embebed in the PDF or annotated through Apple Preview) through a mobile device or regular browser, therefore you will likely need to download and complete on a desktop. If you're not able to do so yourself, I will provide the means for you to sign the forms digitally when you're in the studio. 

• If you elect not to fill out the forms beforehand, we will accomplish this once you're in the studio.

• An intake form is legally required.

• Only one form is necessary, unless otherwise requested in the future.

• You may send it to my e-mail, zdgraney@icloud.com, which is the preferred method, or you may bring a paper copy.
